Hospital Care in your Home

Sometimes the comforts and familiarity of home can speed up the recovery process – so if you fall ill check out your home hospital options too. Many conditions common to those with a SCI are applicable for home treatment, such as cellulitis, severe UTIs, wound care and bowel management.

Step 1.

Get referred by your GP, St John’s Ambulance or your nursing home and Silver Chain’s Priority Response Assessment (PRA) will come and assess you in your home within four hours. Note, this service is applicable only to those in the Perth Metropolitan area.


Step 2.

Once assessed, your best care option will be determined and if your condition can be appropriately treated from home you can enter Silver Chain’s HITH program.

Home Hospital or HITH (Hospital in the Home)

This service enables anyone in the Perth metropolitan area to receive hospital level care in their own home, and is available 24hrs, 7 days a week.

Step 3. 

If it is necessary for you to be admitted to hospital, there are early release options too.

Rehab in the Home (RITH)

With RITH, you can receive short to medium term hospital-substitution allied health therapy at home.

Following a hospital discharge, the multi-disciniplinary RITH team will meet with you at home to continue your rehabilitation, including home assessments, exercise, equipment prescriptions and counselling.
